ehci: fix Interrupt Threshold Control implementation

First, not all interrupts are subject to Interrupt Threshold Control,
some of them must be delivered without delay.

Second, Interrupt Threshold Control state must be part of vmstate,
otherwise we might loose IRQs on migration.
